Patient Forms
Welcome to our practice and thank you for giving us the opportunity to serve your healthcare needs. In order to provide you the most complete service at your first visit it is essential to do the following:
- Please arrive 20 minutes prior to your appointment time.
- Please complete the forms below including personal medical history form and financial policy form completely filled out and signed upon arrival.
- Please bring your insurance card and photo ID so that we may copy them.
- It is absolutely imperative that we have medical records from your primary care physician and/or referring physician. This includes your most recent office notes, lab reports, and pertinent radiology reports. We cannot stress how important this information is for your first visit. Failure to have these records could severely delay your visit. It is always best to bring these records with you, but they can be faxed to
770-962-7868. - If your insurance is an HMO or POS policy that requires a referral you must have that referral with you.
- Please bring the complete listing of all medications that you are taking. Diabetic patients should also bring blood sugar readings for the two weeks prior to the appointment.
