Due to the evolving realities regarding the COVID-19 pandemic, we are making a couple more temporary changes.
DUE TO SOCIAL DISTANCING LIMITATIONS, THERE ARE NO VISITORS ALLOWED. PLEASE DO NOT BRING ANYONE OTHER THAN YOURSELF TO YOUR APPOINTMENT UNLESS YOU REQUIRE PHYSICAL ASSISTANCE OR AN INTERPRETER.
EFFECTIVE September 1st, 2020:
ALL PATIENTS MUST BE SEEN IN THE OFFICE.
However, we are aware that the CDC has determined Diabetic individuals who are the Medicare eligible age of 65 and older are at a higher risk to incur severe illness from COVID-19. Therefore, follow up patients that fall into this category will still be allowed to have their appointment conducted virtually if they prefer.
ALL LAB APPOINTMENTS will continue to be conducted in the office regardless of diagnosis.
PLEASE NOTE: We will still be screening for COVID-19 infection at our entrances, requiring all patients to wear masks and practicing social distancing in the waiting rooms. If for some reason the waiting room is going beyond the capacity regulated by the State’s latest guidelines and you are physically capable, you may be asked to wait in your car until your appointment time and a Medical Assistant will call you when the Provider is ready to begin. Thank you and we look forward to making these great strides towards fully re-opening and getting back to normal as soon as possible.
IF YOU HAVE BEEN ILL OR EXPERIENCING ANY SORT OF RESPIRATORY SYMPTOMS, PLEASE DO NOT COME IN TO OUR OFFICE.
We are limiting the amount of patients allowed in our lobby and there will be “X”s placed on the floor to illustrate the safe distance of where it is o.k. to stand. The front office will not be opening their windows or accepting anything from patients.

For follow up Diabetic appointments being conducted virtually: All you need is an electronic device that accepts email and is enabled with a web cam. Basically, any smartphone, IPad or current laptop/pc should work just fine.
The Way it Works -
- We are reaching out to scheduled patients but please contact us if you have an upcoming appointment and have not heard from our staff. You can simply leave a voicemail, message us through the portal, or message us on Facebook or Google. Please state your name, date of birth and date of appointment. Also please leave the email address where you would like the Provider to contact you. One of our schedulers will contact you to confirm.
- If you have a lab appointment the week prior, you are still required to come in and have your blood drawn. We are keeping our waiting room very sparse and screening patients for COVID-19 symptoms at our entrance.
