EFFECTIVE April 14th, 2021:
In accordance with the evolving state guidelines regarding the COVID-19 pandemic, we are making some small changes to our own current office policies.
Social Distancing - While you will no longer be seeing “X’s” on the floor or blocked off chairs in our waiting room, we still ask that you maintain distance with others when possible. MINORS WILL STILL NOT BE PERMITTED IN THE OFFICE.
ALL PATIENTS, PROVIDERS AND EMPLOYEES ARE STILL REQUIRED TO WEAR MASKS IN OUR OFFICE.
Please note that all patients’ temperatures will still be taken prior to their appointment with a provider. However, we will no longer require temperatures taken upon entry to the office.
Your health and safety are still our top priority, and we will continue with the extra safety measures we have had in place such as additional cleaning and sanitizing in all areas of the Practice.
IF YOU HAVE BEEN ILL OR EXPERIENCING ANY SORT OF RESPIRATORY SYMPTOMS, PLEASE DO NOT COME IN TO OUR OFFICE.

We are a specialty practice that utilizes a teamwork approach to your care. As a patient, you may not see your physician at every visit as we will utilize the services of the Nurse Practitioners, Dietitian and/or the Physician Assistant.
We would like to thank you for the opportunity to assist you in your healthcare needs. In order to serve you best, our physicians require medical records from your doctor that correspond with your need to make an appointment with us. We do not screen for weight loss or weight gain, hair loss or fatigue but we will gladly treat diagnosed conditions with those symptoms.
